A protein-based option designed to target the enzymes associated with joint and cartilage breakdown.
What Is A2M Therapy?
Alpha-2-Macroglobulin (A2M) therapy uses a naturally occurring protein found in your own blood plasma.
A2M is best known for its role as a protease inhibitor, meaning it helps block certain enzymes that are associated with the breakdown of cartilage and other joint tissue. By isolating and concentrating A2M from a small blood sample and reintroducing it into an affected joint or soft-tissue structure, this therapy is intended to help support the joint environment as part of a broader, individualized treatment plan.
How A2M Works
Like PRP, A2M therapy begins with a simple blood draw. That sample is processed to isolate and concentrate the A2M protein, separating it from other blood components. Once prepared, we inject the concentrated A2M directly into the affected joint or soft-tissue structure, often using ultrasound guidance to help precisely target the intended area. Because A2M is derived from your own blood, it is generally well tolerated, and the procedure itself typically takes about as long as a standard in-office injection.
Step 1
Blood draw
A small sample, similar to routine lab work.
Step 2
Concentrate
The A2M protein is isolated from other blood components.
Step 3
Injection
Delivered to the joint, often with ultrasound guidance.
What to Expect During Your Treatment
Most A2M appointments, from blood draw to injection, can be completed during a single office visit. As with any injection, some patients notice mild soreness or swelling at the injection site in the days that follow, which is a normal part of the body’s response.
We will review what to expect afterward, including any activity guidelines, so you know what a typical recovery looks like for your specific situation.
Who May Be a Candidate
A2M therapy is one of the newer additions to the regenerative and orthobiologic options available at Regen Ortho & Wellness, and it may be considered for certain joint conditions, including some cases of osteoarthritis, when appropriate for the individual. Because A2M targets specific enzymatic activity associated with cartilage breakdown, it is not interchangeable with every other regenerative option, and it is not the right fit for every patient.
Figuring out whether you’re a candidate begins with a full look at your diagnosis, imaging, and treatment history, so we can talk with you about whether A2M, another option such as PRP or BMAC, or a combination of therapies best fits your goals.

Why Choose Regen Ortho & Wellness for A2M
At Regen Ortho & Wellness, Nikki Acuff, DMSc, MSPAS, PA-C, evaluates every patient individually before recommending any orthobiologic therapy, including A2M. Rather than offering the same treatment to every patient with joint pain, we will walk you through how A2M compares with other options available at our practice and explain the reasoning behind her recommendation, so you always understand the “why” behind your care.
